What Happens If Volo Removes Your Eye?
After lying down and being allowed to proceed, Volo eventually goes too far when he takes out a larger instrument to use, which seems as if it could have almost killed you.
This ends up with your eye being taken out and you passing out after the surgery that is performed but your character still lives but their eye has popped out.
Volo will make up for this by giving you Volo’s Ersatz Eye, which is an artifact that has magical properties that give improved vision capable of revealing the unseen.
Volo’s Ersatz Eye
Volo’s Ersatz Eye is an item that you can obtain from him after surgery or loot from him that provides you with the ability to see invisible enemies.
This gives you the See Invisibility condition, which will be permanent and the appearance of one of your eyes will be altered.
See Invisibility allows you to see Invisible creatures within a 9-meter radius if they fail to succeed in a Dexterity Saving Throw, which causes them to lose their invisibility.
Downside Of Volo Stabbing Your Eye
There are not that many losses aside from reputation, especially when it comes to Shadowheart as she disapproves of this.
You can always earn her approval later through various acts and this does not make having your eye replaced with a magical artifact that bad.

Verdict
When it comes to having Volo remove your eye, this may appear as something risky at first, but the pros outweigh the cons.
Characters like Shadowheart, Astarion, and Lae’zel will not like you doing this but gaining their approval later won’t be an issue if you are aiming to romance them.
The effects of Volo’s Ersatz Eye can be useful when there are enemies hidden from plain sight, giving you an advantage against them when exploring or in a fight.
